Explaining how to set up a note and what information should be in it to make is sound more professional and reduce risk of being denied. I would have liked more examples of written examples and wish I could print them, I already shared this with my coworkers as some are new grads struggling to create notes and stay within their allowed productivity. It can be challenging. It is easier to use drop downs with the higher productivity standards for assistants.

I am an OTR/L and have worked as a clinical reviewer since 2009, first for work comp claims and the past 4 years for managed care. The information covered was excellent and right in-line with what we look for in documentation. I especially like your concrete examples of validated vs non validated notes and in-depth examples of what notes show skill with the emphasis of not just what the pt is doing but what skilled treatment techniques a being done by the treating therapist in an evolving POC.

I am a relatively new therapist and always feel my documentation is not professional enough. This course helped me to understand some weak areas and gave ideas on how to strengthen them.
